Chronic disease management is one of the most pressing challenges facing Ontario’s primary care system. As more people live with complex and overlapping conditions, the need for proactive, integrated care has never been greater.
Hosted by Longwoods and Best Care, this panel will bring together clinical and health-systems to explore how structured and integrated team-based models can better support patients in the community. Panelists will examine opportunities to strengthen care pathways, improve coordination, and build primary care capacity to reduce avoidable hospitalizations, preventing recurring hospital admissions, and support more proactive, long-term disease management. The discussion will focus on identifying sustainable approaches that improve patient outcomes while easing growing pressures across the health system.

CANCELLATION POLICY:
Leadership Discussion cancellation requests must be submitted in writing to events@longwoods.com.
No-shows occur when individuals register but do not attend the conference and are not eligible for a refund. The Leadership Discussions Planning Committee reserves the right to make changes in programs and speakers, or to cancel the Leadership Discussion if registration targets are not met or when conditions beyond its control prevail. If the Leadership Discussion is not held for any reason, Longwoods Publishing’s liability is limited to the refund of the registration fee only.
SUBSTITUTION POLICY:
If you are unable to attend the Leadership Discussion, you are welcome to send a colleague in your place. There is no fee to make this change. Substitution requests should be sent to events@longwoods.com.
